Life Circle Senior Services provides doctor supported home nursing services to Senior Citizens and bed ridden patients. Life Circle services are centred around a trained nurse supported by family physician and aided by systems and processes required for rigorous and regular monitoring and follow up of an enlisted clientele of senior citizens. The core of the model is a comprehensive health assessment at home for each senior citizen. This is to be done by the nurse supported and guided by the family physician. The nurse is to work with the senior citizen, the care giver, and the family physician to customise an evidence-based comprehensive health plan.
